Carbapenemase-producing Enterobacteriaceae (CPE) surveillance tracks Enterobacteriaceae isolates that harbor carbapenemase enzymes conferring carbapenem resistance, a designation distinct from broader carbapenem-resistant Enterobacteriaceae (CRE) or carbapenemase-producing organisms (CPO) categories. CPE are recognized as an important and increasing threat to global health, driven by both clonal spread and plasmid-mediated transmission [1]. The carbapenemases detected in CPE fall within three Ambler classes: Class A (e.g., KPC), Class B metallo-β-lactamases (e.g., NDM), and Class D (OXA-48-like) [1].

Surveillance entity definition

CPE surveillance is a source-reported epidemiological category focused on Enterobacteriaceae that produce carbapenemase enzymes—β-lactamases capable of hydrolyzing carbapenem antibiotics. Under the Ambler classification, the relevant carbapenemases belong to Class A (KPC), Class B metallo-β-lactamases such as NDM, and Class D (OXA-48-like enzymes) [1]. This surveillance category is narrower than general CRE or CPO concepts and emphasizes genotypic or phenotypic confirmation of carbapenemase production.
