Clinical manifestations are linked to pneumococcal virulence factors, while the surveillance definition itself is based on penicillin susceptibility status rather than a particular clinical syndrome. Pneumococci are reported as frequent colonizers of the upper respiratory tract, with higher amounts described in environments where people spend substantial time in close proximity, including day-care centers and military barracks. Penicillin resistance has increased over time and is especially prevalent in areas with high antibiotic use; resistant strains may also show resistance to several other antibacterial classes. [2]
