Study design

This was an outbreak investigation of 469 COVID-19 cases linked to multiple summer events and large public gatherings. Genomic sequencing was performed on specimens from 133 patients, and RT-PCR cycle threshold values were compared between vaccinated and unvaccinated groups.

Population and setting

The study included Massachusetts residents who attended large public gatherings in a town in Barnstable County during July 2021, when vaccination coverage among eligible residents was 69%.

Main findings

Of 469 cases, 74% occurred in fully vaccinated persons. Genomic sequencing identified the Delta variant in 89% of sequenced specimens. Among vaccinated individuals with breakthrough infections, 79% were symptomatic. RT-PCR cycle threshold values were similar between vaccinated breakthrough cases (median 22.77) and unvaccinated or not fully vaccinated persons (median 21.54). Four of five hospitalized patients were fully vaccinated, and no deaths were reported.
