百日咳的潜伏期通常为七至十天，初期症状类似普通感冒，表现为轻度发热、流涕与轻微咳嗽[2][1]。随后病情进展为阵发性剧烈咳嗽，可持续一至六周甚至更久，咳嗽发作后常伴有特征性的高调吸气性哮声[2][1]。剧烈咳嗽可导致疲劳、呕吐及肋骨骨折；一岁以下婴儿可能缺乏典型咳嗽而仅表现为呼吸暂停[1]。此外，肺炎是相对常见的并发症，惊厥与脑部病变则较为罕见[2]。
