Study design
An indirect cohort vaccine-effectiveness study using clinical and epidemiologic information and pneumococcal isolates collected through enhanced invasive pneumococcal disease surveillance in Japan from April 2013 through December 2017. Adults older than 20 years were included.

Main findings
Adjusted PPSV23 effectiveness against PPSV23-serotype invasive pneumococcal disease was 42.2%. The proportion of 13-valent PCV serotypes declined from 45% to 31%, while estimated effectiveness against PPSV23-serotype disease changed from 47.1% to 39.3%; in elderly patients, the change was marginal, from 39.9% to 39.4%. The pediatric PCV program was assessed as having limited impact on adult PPSV23 effectiveness.
