登革热是一种自限性发热性疾病，临床谱系从无症状或轻微发热（登革热，DF）到严重出血热（登革出血热，DHF）乃至致命的登革休克综合征（DSS）不等[1][2][3][4]。典型症状常在受感染蚊虫叮咬后约4–10天出现，初期表现类似流感，包括发热、全身不适等；重症患者可出现呼吸窘迫、鼻和牙龈出血以及血压急剧下降乃至休克[1][2][4]。常见的实验室异常包括血小板减少、白细胞减少和血管通透性增加，重症机制涉及抗体依赖性增强（ADE）、免疫失调、病毒毒力及宿主遗传易感性等因素[4][5]。
登革热在热带和亚热带地区广泛流行，已在逾百个国家报告，估计每年全球发生约4亿例感染和约22 000例死亡[4]。早期估计每年约有1亿例登革热、50万例登革出血热和25 000例死亡，是仅次于疟疾的最重要热带感染病[3]。其流行具有明显的季节性，传播高峰常出现在雨季期间及雨季后，与蚊虫种群密度、循环血清型易感人群、气温、降水和湿度等因素密切相关[1]。在过去二十年中，病毒和蚊媒的地理分布不断扩大，多血清型共同传播形成的高度地方流行性增加，登革出血热在新的地理区域出现，疫情的强度、频率、持续时间和分布范围均因城市化、气候变化和持续性监测控制措施不足而加剧[1][3]。
登革病毒主要通过受感染雌性伊蚊（Aedes spp.）叮咬传播给人类，每年数百万人因蚊虫叮咬而感染[2][4]。蚊媒种群数量、易感人群比例以及气温、降水和湿度等环境因素会影响病毒的繁殖、蚊虫吸血模式及病毒在蚊体内的潜伏期，从而影响传播动态[1]。
目前尚无普遍获批且广泛使用的特异性抗病毒药物或普适性疫苗，第一个登革疫苗为四价CYD Denvaxia，但并非在所有国家获批，且缺乏合适动物模型、对发病机制的了解以及抗体依赖性增强等问题阻碍了疫苗开发[2][4][5]。因此预防策略主要依赖蚊媒控制、个人防护以及针对蚊虫孳生地的环境治理，结合支持治疗和重症监护以降低死亡率[1][2]。
