疟疾是一种由疟原虫属寄生虫引起的急性发热性疾病，主要通过受感染雌性按蚊叮咬传播，集中分布于热带地区，对全球公共卫生构成持续性挑战[1][2]。2024年全球估计有2.82亿例疟疾病例和61万例死亡，疫情在80个流行国家持续存在[1]。儿童尤其是5岁以下儿童承受最沉重的疾病负担，非洲区域集中了绝大多数病例与死亡[1][3]。及时诊断与有效治疗以及媒介控制与化学预防相结合的综合策略对降低疾病负担至关重要[4]。

疟疾的临床表现轻重不一，轻症可表现为发热、头痛和寒战，常与其他发热性疾病难以区分[1][2]。在无免疫力的个体中，症状通常在感染性蚊虫叮咬后10至15天出现；若未及时治疗，恶性疟疾可在24小时内进展为重症甚至死亡[3]。重症表现包括疲劳、意识模糊、抽搐和呼吸困难；儿童重症患者常见严重贫血、与代谢性酸中毒相关的呼吸窘迫以及脑型疟疾，成人则常出现多器官衰竭[1][3][5]。
2024年全球估计有2.82亿例疟疾病例和61万例死亡，疫情波及80个国家，其中WHO非洲区域承担了不成比例的沉重负担，集中了约95%的病例（2.65亿）和95%的死亡（57.9万）[1]。WHO东南亚、东地中海、西太平洋及美洲区域亦报告大量病例，2021年估计84个流行国家共有2.47亿例和61.9万例死亡[3][4]。在非洲区域，5岁以下儿童占疟疾死亡的近75%；近年来多个非洲和南美洲国家还出现疟疾疫情反弹，部分受COVID-19相关卫生服务中断影响[3][4]。
疟疾主要通过受感染的雌性按蚊叮咬传播给人，不存在人传人的直接传播途径[1][2]。全球已记录超过400种按蚊，其中约40种可作为传播媒介，不同地区的传播风险因当地蚊媒种类和季节而异，热带国家的雨季风险最高[2]。此外，受污染的针头和输血也可能造成传播[1]。
疟疾的预防主要依靠避免蚊虫叮咬以及合理使用预防性抗疟药物[1]。前往疟疾流行区的旅行者应提前数周咨询医生以确定适宜的化学预防方案，并按计划在停留期间持续服药，离开风险区后还应继续服药4周以覆盖肝脏期寄生虫可能释出的时间窗口[1]。WHO推荐的间歇性预防治疗（孕妇与婴儿）、季节性化学预防以及2021年和2023年获得推荐的针对红细胞前期疟疾疫苗等干预措施，已被证实可显著降低疟疾发病[4]。
