细菌性梅毒
Syphilis
梅毒是由梅毒螺旋体引起的细菌性性传播感染[1][2][3][4]。该病可经性接触、血液及母婴垂直传播，未经治疗可历经多个阶段并造成包括先天梅毒与死胎在内的严重后果[1][2][3]。多数感染者症状轻微或无明显症状，疾病可通过安全性行为预防并以抗生素治愈[1][2][3]。

梅毒的临床表现依病程阶段而异：初期以单个质硬、无痛、非瘙痒性溃疡（下疳）为典型表现，可伴有多处病灶[1][2]。二期梅毒以弥漫性皮疹为特征，常累及手掌与足底，并可出现口腔或阴道黏膜损害[1][2]。潜伏期梅毒无明显症状，可持续多年[1][2]。三期梅毒可出现树胶肿、神经系统损害及心血管症状[1][4]。神经梅毒可在任何阶段发生，导致脑膜炎、葡萄膜炎、听力丧失或卒中[4]。
2022年全球估计有约800万18至49岁成人新发梅毒感染[4]。2012年数据显示约0.5%的成年人受到感染，每年新发病例约600万例，而1999年估计新增感染约1200万例，其中超过90%的病例发生在发展中地区[1]。该疾病每年影响70万至160万次妊娠，导致流产、死产和先天梅毒[1]。2015年梅毒造成约10.7万人死亡，较1990年的20.2万有所下降[1]。在撒哈拉以南非洲地区，梅毒导致约20%的围产期死亡[1]。2019至2023年美国梅毒病例总体增加61%，女性诊断增加112%，先天梅毒病例增加106%[4]。
梅毒主要通过阴道、肛门或口交时接触感染性病灶而传播，也可经输血及妊娠期胎盘垂直传播给胎儿[1][2][3][4]。初期梅毒通过未受避孕套充分覆盖的下疳即可发生传播[2]。
正确并持续使用乳胶或聚氨酯避孕套可降低性传播风险[1][2]。然而梅毒仍可通过避孕套未覆盖的身体部位（包括生殖器、肛门和口腔）传播[2]。高风险人群应至少每年进行一次检测[2]。孕妇应在首次产前检查时接受梅毒筛查，一旦结果阳性应立即治疗；先天梅毒只能通过在孕期内使用青霉素诊断并治疗母体来预防[2]。已诊断梅毒者应通知其性伴侣以预防新发感染[2]。
